Comprehensive End of Life Planning Package
Offers support for the "business of dying"- Advance Directive Planning: assistance with creating advance directives, living wills, and POLST forms to document medical wishes. Guidance on choosing a healthcare power of attorney and outlining decisions regarding life support, resuscitation and palliative care.
- Financial and Legal Preparation: Support in organizing important documents like wills, trusts and financial account information. Assistance with finalizing legal details, such as selecting an executor, ensuring beneficiaries are up to date and providing access to accounts and insurance policies. Funeral preferences, financial organization, and legal document preparation.
- Funeral and Memorial Planning: Guidance in making funeral or memorial arrangements, including preferences for burial, cremation or green burial options, help with writing obituaries, arranging services and coordinating with funeral homes or other venues.
- Practical Preparation for Loved Ones: Organization of critical information, such as passwords, account numbers, and access to personal assets. Creating checklists for loved ones to guide them in tasks immediately following death, including whom to notify and necessary steps.
Comprehensive Death Doula Package
Holistic support for both, the dying and their loved ones throughout the end-of-life journey- Emotional and Spiritual Support: Regular 1:1 sessions to provide a compassionate presence, addressing fears, anxieties and helping the person reflect on life and death.
- Spiritual or Ritual Support: Custom rituals, prayers, meditations, guided reflections and/or energy work such as Reiki.
- Advance Care Planning: Guidance in documenting personal wishes for medical care and decisions about treatments, life support, comfort measures, including advance directives and POLST forms.
- Legacy Work/Projects: Helping clients create meaningful legacy projects, like writing letters to loved ones, recording stories, or crafting memory items to leave behind for family and friends.
- Bedside Vigil and Transition Support: Sitting vigil during the final hours or days, offering comfort, helping create a calm atmosphere with music, candles, aromatherapy, intuitive energy healing and ensuring any last wishes are respected. Performing or guiding simple rituals at the bedside.
- Community Support: Providing emotional support and grief resources to family members before, during and after their loved one’s passing. Serving as a liaison with healthcare providers, helping the family understand options and ensuring the loved one’s wishes are respected. Assisting families with post-death needs, including care of the body (if requested), coordinating with funeral homes, and offering guidance on memorial services or home funerals.
